Presenting Complaint:
Numbness in hand following trauma

Past Medical History
• Recent admission with raised liver markers and muscle inflammation (CK elevated)
• Trauma with extensive bruising to one side of body
• Memory loss regarding traumatic incident

Diagnosis:
Post-traumatic peripheral neuropathy - query nerve injury

I had the pleasure of meeting Lauren Goodinson Powell in Same Day Emergency Care today.

Ms Goodinson Powell presented with numbness affecting her hand following recent trauma. She reports hitting her shoulder and hip, with extensive bruising down one side of her body, though she has no memory of the actual incident. The numbness is described as similar to the sensation of having "sat on your hand" with tingling sensations. She experiences pain on certain movements but maintains full power and function in the affected hand.

On examination, she has preserved sensation to light touch and maintains full motor function including thumb opposition and grip strength. There is localised tenderness over the affected area but no weakness detected. The distribution appears consistent with a peripheral nerve involvement.

Her recent blood tests show improvement with liver markers still elevated but trending downward. The CK (creatine kinase) which was previously raised during her hospital admission is also coming down towards normal levels. Liver ultrascan has returned as entirely normal.

Investigations:
Blood tests: Liver markers raised but improving, CK elevated but trending down
Liver ultrasound: Normal

Plan:
1. Neurology consultation for assessment of peripheral neuropathy
2. Consider nerve conduction studies if recommended by neurology
3. Continue monitoring liver function and CK levels
4. Discharge home with neurology follow-up arranged
