Pamela noon 1116510 74 year old lady recently admitted with giant cell arthritis and on boatloads of steroids for that persistent iron deficiency for some time but note developing microcytosis recently and hemoglobin a 120 negative fit and negative ttg no weight loss no gi symptoms apart from had long standing ibs is fit for upper and lower gi investigations plan for blood test today gv has tried iron supplementation but patient not tolerating because of worsened ibs symptoms plan but to stay if she's anemic then she can have investigations for ida but if she's not anemic then I think we put the whole thing to bed and say it's just iron deficiency without anemia she's got another reason to become anemic though which is anemia of chronic disease because of her inflammatory state so I think it's bit of a challenging 1 she's also really tough
