60 and how many per day never more than 20 but I work had done for the last year down to 7 okay well we could be sort of between 50 and 60 maybe a day for 60 years years ago yeah okay okay until recently I've done a bit so quite a bit of sort of smoking in territory and when you're otherwise well say before you came into hospital and you had all these things going on with your throat what was your breathing like at that time the last since last october you know short of breath but is that when you stopped me doing anything okay so you feel a bit short of breath since october when you exert yourself yeah going up the stairs that kind of thing I live in a bunker oh you live a bunker there okay alright yeah so but I do a job and then now I have to take a little rest and then so if you go back to october I appreciate this is sometimes difficult to remember how far could you walk with before you became short of breath to the. That you'd have to stop quite a bit okay yes sort of to the shops you know how how what would you be doing yeah yeah we do a lot down there okay so loads of walking and you were never really and then how has that changed then it changed all of a sudden I felt like a tightening in my chest mhmm and blood pressure was very high yeah and this is what you noticed last year that's what I noticed so I went to the gp hines yeah a week apart the blood pressure and then it was high so he sent the doctor Scott referred me to that yeah you had this angiogram didn't you this of your heart yeah and they put some dye around and they were gonna put a stent in weren't they and they said maybe in a couple of years yeah you might need a stent to put it yeah and at that time then you were getting tightness in your chest I don't get it very often okay these attacks take a lot but when they do come about couple of minutes yeah you know until I take the spray and then and that eases and at that time were you also along with the tightness were you also finding that you were short of breath no not really no but in terms of the shortness of breath you think that's got worse since october in terms of the amount that you can do yeah before you need to stop yeah has got gradually less since october until now yes and now you're feeling that you're limited by your shortness limit myself to do anything okay yeah but I know that okay I need to have a rest sure not very long maybe 10 minutes go to an hour and then I'll carry on again you must get your breath back yeah I'll let my hands clean yep that's good and do you cough do you have a cough yes that is the only thing that is how long have you had a cough for 3 4 months kind of there all the time only when it's booked only when you smoke no when you cough what do you bring up it's quite a bit of phlegm what color ever any blood no no k and do you think that cough over time has got worse it baps yeah okay you know comes and goes I might have a bath for 10 minutes and then it starts sure can you lie flat in bed at night I just lie flat don't wake up gasping for breath no no any pain in your legs or any swelling in your legs this left 1 it's been quite effective is swollen under there we go okay okay let me have a look at that have you not had any sharp pains in your chest no have you ever had a clot before no not just and anyone in your family ever had a problem with clots my mother had a stroke so just to get things correct so I'm understanding correctly go back to october from then until now you've had increasing shortness of breath when you exhale yourself and the amount that you can do before you get shortness of breath has got gradually less yeah for a few months you've also had a cough bringing up whitish phlegm with no blood okay have you do you ever feel wheezy no no no and those chest pains that you were seeing cardiology about how are they now are they better they don't I don't get them very often no thank goodness you know could be 3. Yeah this thing on your finger when you're measuring your oxygen levels have you ever had that done before yes it's always never been maximum if you know what I mean well what when you say do you know what numbers though you've you've had before okay never a 100 no that's 1 neither are for you no no it's okay that's okay and and do you do you have them at the gp yes okay fine do you mind if I have a listen to your chest it's very I don't know whether it's something that's don't that's I don't if you're speaking in full senses you only go down to 58 yeah yeah and then you know you don't seem short of breath oh and I completely understand the conundrum from the and es and throat team that the numbers look bad but you look good look at your nails because my mouth could get up there were tiny nails I found bronchiepsis on the ct and bronchiepsis oh was that mhmm and feel me clap clap clap clap clap clap clap yeah do you mind if I just undo this 1 just so I can I'm gonna turn this on mind you k alright just do the top bit I can work around to you I'll have a listen I'm gonna listen for that first you could can you take some open breaths on the left for your right so look at his legs you see there's a bit more swollen on the right isn't it sort of the whole leg really yeah do have any pain in your belly no any pain when I press your calves there think it's probably not the same how long have you noticed this leg being more swollen been about 4 weeks 4 weeks have you had pain in this knee no it's in leg down little warmer a little bit
