Reason for Call:
Emergency call for patient who experienced transient loss of consciousness and hypotension during dialysis session.

A-E Assessment:
A - Airway: Patent, no interventions required

B - Breathing: Patient stable, no respiratory distress noted

C - Circulation: Initially hypotensive during dialysis with transient loss of consciousness. Blood pressure improved following cessation of dialysis. Had completed 2.5 hours of planned 3-hour dialysis session when episode occurred.

D - Disability: Transient reduced GCS during hypotensive episode, fully resolved post-dialysis cessation. No ongoing neurological concerns.

E - Exposure: Afebrile, dialysis access lines intact

Investigations:
ECG - Performed post-episode
Troponin - Taken at time of episode, result pending
Routine bloods - Taken post-dialysis

Impression:
Transient hypotension and loss of consciousness secondary to dialysis-related fluid removal. Episode resolved with cessation of dialysis. Patient had received 2.5 hours of planned 3-hour session.

Plan:
- Dialysis session terminated early due to hypotensive episode
- Close monitoring for further episodes
- Patient advised to report any chest pain immediately
- ECG and troponin monitoring
- Blood results to be reviewed
- Patient stable for discharge to ward care with routine observations
