CLINICAL NOTE

Patient underwent uncomplicated upper GI endoscopy today without biopsies for investigation of heartburn and reflux.

HISTORY:
Patient missed morning antihypertensive medication (Lisinopril 20mg) taken for essential hypertension.

POST-PROCEDURE COMPLICATIONS:
Following endoscopy, patient developed:
- Headache
- Vomiting (recurrent)
- Chest tightness
- Shortness of breath
- No chest pain
- No oxygen requirement

OBSERVATIONS:
- Heart rate: 90 bpm
- Blood pressure: 234/105 mmHg
- Oxygen saturation: Normal
- ECG: T wave inversion V1-V3

EXAMINATION:
- Chest: Clear
- Heart sounds: Normal
- No peripheral oedema
- Appears unwell and uncomfortable with recurrent vomiting

PLAN:
1. Antiemetic administration
2. Recheck ECG in 30 minutes
3. Consider CT angiography to exclude aortic dissection as cause of hypertension
4. Transfer to Medical Receiving Unit for further review and assessment of symptomatic hypertension

IMPRESSION:
Symptomatic hypertension secondary to missed antihypertensive medication. Exclude aortic dissection as underlying cause for hypertension.
