Reason for Call:
Medical emergency call following crash bell pulled on Simpson Ward. 90-year-old lady admitted with symptomatic anaemia thought secondary to DOAC. Hb on admission 61 (previously 68 with GP on 28th January).

A-E Assessment:
A - Airway: Patent

B - Breathing: Chest clear bilaterally

C - Circulation: Blood pressure satisfactory although slightly tachycardic

D - Disability: Responsive to voice, GCS 14 on team's departure

E - Exposure: Abdomen soft and non-tender, no evidence of active blood loss

Investigations:
Blood tests including repeat group and save, venous blood gas (reassuring), 12 lead ECG, cross match 2 units of blood in case required depending upon results of full blood count

Impression:
Patient reports feeling exhausted with no chest pain. Possible recurrent symptomatic anaemia in frail elderly lady.

Plan:
For discussion with family regarding ongoing management. Not for resuscitation status discussed and agreed with family. Blood products available if required based on FBC results.
