Evidence of IDA: long-standing anaemia (at least 2020) Progressive microcytosis over the last year with slightly elevated platelets and ferritin 224,

• Long-standing anaemia since at least 2020-2021, required iron infusion pre-operatively for hip surgery
• Progressive microcytosis over the last year with slightly elevated platelets and ferritin 224 (but 121 in July)
• FIT was 22
• No GI symptoms, stable weight
• Started Edoxaban 2 years ago for AF (begin of MCV decrease)
• Main symptom: breathlessness limiting to 20 steps
• Lives independently, drives, minimal alcohol, non-smoker
• Recent BNP >4,000 indicating LV failure contributing to breathlessness
• No reported bleeding
• Malignancy risk 11.2% (age-related, duration of anaemia reassuring)
• O/E: Height 164cm, Weight 56.75kg, BMI 21.1

Plan was
• Upper GI endoscopy with sedation
• CT abdomen and pelvis with 2 days Gastrografin prep (lower GI study)
• Blood tests for TTG
• Patient needs escort for endoscopy if sedated
