Reason for Call:
Collapse following mobilisation - patient became anxious and shaky when attempting to walk, developed neck pain, then completely passed out.

A-E Assessment:
B - Breathing: Saturating 99% on air, breathing maintained throughout episode

C - Circulation: Blood pressure 120/30 initially, later noted as 160/70 baseline with drop to 129 during mobilisation. Heart rate increased. Clinical concern for postural hypotension.

D - Disability: Complete loss of consciousness lasting approximately 3 minutes. Patient kept eyes closed but maintained spontaneous breathing. Returned to baseline neurological state.

E - Exposure: Patient appeared dehydrated. Abdomen slightly distended with reduced bowel sounds, though history of regular bowel movements reported.

Investigations:
Full set of bloods sent
Chest X-ray requested
ECG performed - T wave inversions noted inferiorly, query new changes
VBG requested

Impression:
Collapse secondary to postural hypotension, likely multifactorial:
- Dehydration
- Medication effect (patient on Doxazosin)
- Possible electrolyte disturbance

Plan:
- Rule out electrolyte/cardiac/infectious aetiology
- Review blood results and imaging
- Review Doxazosin dosing and blood pressure management
- Anti-emetic PRN
- Ongoing monitoring by ward team
- Repeat ECG if clinical concern
