Main issue was developing microcytic anaemia with change in bowel habits.

Mr Newman is an 83-year-old gentleman who remains fit and active, living independently in a flat, doing his own shopping and pursuing active hobbies including jewellery making and woodwork. His past medical history includes mild COPD, hypertension, and type 2 diabetes. He denies any symptoms of anaemia or gastrointestinal symptoms including blood loss. However, he has experienced a change in bowel habits over the past 6-12 months and now requires daily laxatives. His weight remains stable or increasing. He has longstanding heartburn, well controlled with omeprazole.

Investigations reveal a FIT of 53 micrograms per gram, developing microcytosis and anaemia in combination with low ferritin. Given the combination of microcytic anaemia, low ferritin, elevated FIT, and change in bowel habits in an elderly gentleman, there is concern for possible gastrointestinal malignancy.

Plan was upper and lower GI endoscopy given his good functional status and likely tolerance of procedures.
